How do I qualify for BCHP?

It is easy to qualify for BCHP services.

1. You must be a woman;
2. You must be between the ages of 40 and 65 years of age;

3. You must have an income that is at or below 250% of the federal poverty level.; The income chart will help you make that determination.

You can have health insurance and still qualify too.

1. You can qualify if your insurance is major medical and does not pay for preventative care;
2.
You can qualify if your insurance has a deductible that is more than $100.00 and you have not paid down the deductible yet.

If you qualify for the Breast and Cervical Health Program all you need to do is contact one of the many regional primary care providers. When you call the provider tell them that you want to enroll in the Breast and Cervical Health Program and they will take it from there.
